New Patient Qualifications
All new patients are required to qualify for services. Please review the following information below to see if you qualify.
Must fit within income guidelines
To qualify for any of our dental services, you must fit within our income guidelines. This is generally 200% below the poverty level.
Must be 18 or older
To receive any of our dental services, you must be at least 18 years or older. It is our goal to provide dental services to anyone under the age of 18 but unfortunately, we are not there yet.
Required Documents
All new patients are required to have approved documentation. Please review the following information below.
Must have proof of residency
We require some sort of proof of residency. This could be in the form of a lease, rent agreement, or a current utility bill in your name.
Must have proof of income or financial support
This could be in the form of a recent paystub.
Must have valid drivers license or ID
We ask that new patients have a valid drivers license or identification card.
