To qualify for any of our medical services, you must fit within our income guidelines. This applies to individuals who fall at or below the 200% poverty level.
To receive any of our medical services, you must either have no insurance or a high deductible, such as $5,000 or more. We will accept individuals with Medicaid who are unable to access a provider.
All new patients are required to have approved documentation at the time of the prescreening intake appointment. Please review the following information below.
We require some sort of proof of residency. This could be in the form of a lease, rent agreement, or a current utility bill in your name.
This could be in the form of a recent paystub.
We ask that new patients have a valid drivers license or identification card.
Examples include: birth certificate, Medicaid cards, report cards/current school records, or immunization records.
